Guaranteed issue (GI) for Medigap (Medicare Supplement) means that, in defined situations, an insurer generally must sell you a Medigap policy without medical underwriting—and with limits on pre-existing-condition waiting periods—even if you are outside the six-month Medigap open enrollment window that starts when you are 65+ and enrolled in Part B. GI rights are narrower and trigger-specific. They are not a permanent open market for every letter at every age. They are also separate from Part B/Part D late-enrollment penalties, IRMAA, and Medicare Advantage annual election periods.

Exact lists live on Medicare.gov and change with statute and guidance. Situations people often research include:
- Losing certain employer or union coverage that paid after Medicare.
- Your Medigap issuer goes bankrupt or no longer offers coverage in defined ways.
- You leave Medicare Advantage under a trial right and want to return to a Medigap policy you had (or, in some trial paths, to a Medigap policy) within the trial window.
- Other loss-of-coverage events listed in CMS materials where Medigap GI is explicitly granted.
GI is not the same as “I dislike my Advantage MOOP so I want Plan G tomorrow.” Without a listed trigger (or state protection), underwriting may apply.

Worked example: trial right vs underwritten jump
Chris drops a Medigap Plan G at age 67 to try a Medicare Advantage PPO for one year under a documented trial right. Eleven months later Chris wants Original Medicare + Medigap again. Using the trial-right GI path on time, Chris can often return to Medigap under the rules that apply to that trial—without the same underwriting wall a neighbor faces.
Chris’s neighbor Dana, also 67, simply applies for Plan G three years after Medigap OE with no GI event and a recent cardiac procedure. Two issuers (illustrative: UnitedHealthcare and a regional Blue) decline; one offers a different letter after underwriting. Dana still owes Part B premiums and faces more of Medicare’s cost-sharing without a supplement. Calendar discipline on GI deadlines would have mattered more than shopping brand logos.
Practical sequencing
- Write down the event date (coverage loss, trial-right start/end, issuer notice) and count the GI application window in days.
- Ask state SHIP which letters GI covers in your state for that event.
- Compare the same letter across issuers; benefits match in most states, premiums do not.
- Keep Advantage trial-right paperwork separate from the six-month Medigap OE calendar.
- Budget Part B and any IRMAA separately from Medigap premiums.
- Buy only through licensed agents / official issuer channels—Medigap phishing is common.
